It has always been my dream, passion, and honor to practice medicine and serve my patients like family. Unfortunately, we are now faced with an increasing number of provided services and resources that are not covered by private insurances of Medicare. In addition, the recent pandemic has necessitated further adaptions in how we deliver patient care including the implementation of telemedicine. In order to continue providing you the level of care we expect and that you deserve, the following three-tier office model was introduced to our patients and will continue in 2025.

  • Annual fee: $495 for Nephrology/ $950 for Internal Medicine

    Designed to support patients who value coordinated, organized care beyond the traditional office visit.

    Includes:

    • Physician-directed coordination with hospitals, specialists, and outside providers

    • Review and integration of outside medical records into your care plan

    • Prescription management support, including refills and insurance authorization assistance during

    business days and hours

    • Follow-up coordination for labs and diagnostic testing when additional clarification or care planning is

    required

    • Secure electronic access to your medical record including results of labs and tests ordered

    • Discussion of abnormal results requiring changes in management will require separate tele-health or in-

    person office visits

    For patients who are not Medicare beneficiaries:

    Enrollment in one of the care programs is required in order to continue receiving care through our internal

    medicine practice.

    For Medicare beneficiaries:

    Participation in the Basic Care Coordination Program is optional. Medicare patients who choose not to enroll

    will continue to receive all covered services without limitation.

  • Annual fee: $4000

    Designed for patients who desire more timely access and a higher level of proactive communication.

    Includes all Basic services, plus:

    • Secure electronic messaging responses or telephone/tele-health responses within 48 hours

    • After-hours and weekend prescription requests and non-urgent clinical questions

    • Priority communication regarding laboratory and diagnostic results

    • Office or telemedicine appointments typically within 1–2 business days

    • Waiver of missed appointment fees

  • Annual fee: $7500

    Intended for patients who value the highest level of physician access, continuity, and involvement.

    Includes all Enhanced services, plus:

    • Direct physician access via phone, email, or telemedicine

    • Same-day or next-business-day appointments with your physician when needed

    • Proactive care planning and hospital-to-office continuity

    • Expedited coordination of referrals and complex care needs

    • Comprehensive annual wellness and preventive planning evaluations

    Patients who choose not to enroll and are not Medicare beneficiaries will be provided reasonable advance notice

    and assistance with transition of care.

    Our staff is available to answer questions and assist you in selecting the option that best fits your needs.

    My commitment remains unchanged: to provide thoughtful, personalized medical care while adapting to a

    healthcare environment that no longer supports this level of service through insurance alone.
